HDD-OJEULT-56169- Assisted Discharge Low Level Community Support (Pembrokeshire)

The Pembrokeshire Integrated Assisted Discharge and Home Support Services aims to prevent unnecessary admission to hospital and facilitate early discharge from hospital and is a critical component of demand management from the perspective of the hospital and social care teams. The vision of the service is to utilise a range of third-sector intermediate care services, to provide a range of support, avoiding the need for conveyance to hospital and enabling people to stay safely in their own home. The service is not a regulated care service and therefore is designed to meet non personal care needs based on a person-centered assessment of needs. The staff supporting the service and conducting the assessment not only consider the health & care needs, but also look at the wider determinants of wellbeing, signposting to third-sector services, addressing issues such as loneliness and social isolation, financial hardship, and so on. This service does not operate in isolation or seek to duplicate other third sector services in operation but aims to leverage support and refer to other services where appropriate.

Framework Agreement for the provision of Children and Families Supported Outreach

The Council of the London Borough of Redbridge (“the Authority”) sought Tenders from suitably experienced and qualified organisations for the Framework Agreement for the provision of Children and Families supported outreach services. The service shall commence on 1st March 2024 and shall continue up to and including 29th February 2028 (“the Contract Period”). Flexible Framework for Services linked to Scottish Attainment Challenge (SAC) and Pupil Equity Fund (PEF) - Entry Point 3

The Council have already established a Flexible Framework (https://www.publiccontractsscotland.gov.uk/search/show/search_view.aspx?ID=SEP460524)for services linked to Scottish Attainment Challenge and Pupil Equity Fund. . This notice is for Entry Point 3, which is applicable for any potential Provider and/or setting that is not already on the Framework. . The original Framework was set up for 4 years, with included annual entry points to allow new Providers to apply to join the Framework, this would enable the Council to maintain capacity and choice. . Key objectives of Scottish Attainment Challenge is to provide opportunities for local authorities in addressing the poverty related attainment gap. . The Pupil Equity funding is additional funding allocated directly to schools with objective to allow flexibility within a local context for schools to target interventions aimed at closing the poverty related attainment gap. . It is envisaged that Service Providers appointed to this Flexible Framework will support these objectives. The resultant services will be made available to Primary, Secondary and Additional Support Needs schools within NLC.

Pupil Equity Funding Framework

The Scottish Government committed additional funding in the form of Pupil Equity Funding (PEF), to schools as part of the Scottish Attainment Challenge programme. This funding is allocated directly to schools and is aimed at closing the poverty related attainment gap. The aim of this framework agreement was to allow schools to contract compliantly with providers who would be able to support the pupil equity fund.

AWARD of the Flexible Purchasing Framework The City of Edinburgh Council has created a Flexible Purchasing Framework of providers in line with the National Improvement Framework for Education. The vision for education in Edinburgh is to ensure excellence by raising attainment and improving outcomes through Accredited Pathways and Certified Counsellors and Therapists: ensuring that every child and young person achieves the highest standards in literacy and numeracy, as well as the knowledge and skills necessary to shape their future as successful learners, confident individuals, responsible citizens, and effective contributors. The City of Edinburgh Council will realise this vision by achieving equity: ensuring every child and young person has the same opportunity to succeed, regardless of their background, social circumstances, additional needs or shared protected characteristics, with a particular focus on closing the poverty related attainment gap, and improving mental health and wellbeing. In order to achieve this, The City of Edinburgh Council will work to reduce the Poverty Related Attainment Gap (PRAG) through a transdisciplinary approach. Delivery of excellence will be achieved via the Edinburgh Imperative, which has four key themes to support reduction of the PRAG: Attendance, Literacy & Numeracy, Pathways, Health and Wellbeing. Collaboration and relationship building with partner providers is a key theme to support young people’s outcomes being at the heart of everything we do. Partner Providers will support schools to provide a service which ensures pupils receive high quality education which meets their needs, delivers improved educational outcomes and avoids exclusion. The City of Edinburgh Council are encouraging a ‘Place Based’ approach to services and supports to families and young people. The intention is to develop comprehensive supports that are within 20minutes of any identified need.
