Description

This often leads to health problems and social exclusion being exacerbated and the potential to rely on crisis or statutory services and high-cost care later in life. Citizens in scope will be experiencing or have experienced two or more of the following: •	substance misuse;•	poor mental health;•	a history of repeat homelessness; •	a history of offending;•	those who experience serial exclusion from services or are at risk of exclusion from mainstream services.
