Description

Peripatetic Care workers to extend delivery of additional Pathway 1 capacity identified following Demand and Capacity Modelling and intermediate care Plan. This is to enable Phase 2 Procurement, Commissioning Intentions and Implementation to be completed whilst ensuring the Plymouth system has sufficient capacity to support flow and discharge out of acute and community settings, preventing unnecessary spend on spot purchased on pathway 2 capacity as part of our delivery of the intermediate care plan.

NHS Norfolk & Waveney ICB (NWICB) seeks competitive bids for the Provision of a Learning Disability Respite Service for People aged 18+ with a learning disability, who are under the duty of care of NHS Continuing Healthcare (CHC) and are registered with a GP within Norfolk & Waveney.<br/><br/>The successful Provider will deliver a five-bed residential respite service with nursing care for adults with learning disabilities and complex health needs.<br/><br/>The service will be provided 24 hours per day, seven days a week including days, evenings and weekends, and will be provided over a full 52 weeks per year, as defined and detailed in the Essential Standards set by the Care Quality Commission (CQC). Except for closure for staff training (4 days per year) and Christmas closure arrangements to be agreed with the nominated liaison team manager who oversees referrals to the service.<br/><br/>This contract will be for an initial 3 year contract period, with an option to extend up to a further 24 months subject to Commissioner discretion and requirements (5 years in total). Habilitation Services for Children with Visual Impairments

The service will work with babies, children and young people who are blind or have significant visual difficulties which may be ocular, cerebral or both ocular and cerebral, which will inhibit their ability to travel and live independently. The service provider will work in conjunction with Swindon Borough Councils Vision Support service, who will provide the referrals. The initial contract term will be for 12 months as stated in the Request For Quotation documents. Estimated contract spend is capped at £50,612.80 across the 12 month contract term.
