SANDWELL PROMOTING INDEPENDENCE REABLEMENT SERVICE

Sandwell Metropolitan Borough Council wishes to appoint up to 10 Providers for the provision of a Reablement Service to eligible individuals living in Sandwell. All Providers will be appointed to deliver the Service across the whole of the Borough of Sandwell. It is anticipated that the Service will support approximately 2000 hours of care per week, however the amount of hours of care per week may vary during the duration of the Agreement. The Service will provide Reablement services to eligible adults (18 years old and over) aimed at enabling individuals: • To safely leave hospital where they would otherwise remain in hospital or avoid a hospital admission and remain in their own home or place of residence; • Provide a short term, time limited (up to 6 weeks) Reablement Service for Adults who have been assessed and have demonstrated the potential to benefit from home care reablement to regain their independence and live well at home after an illness, injury or period of reduced mobility;

Innovative Platform for Home Based Services

Under the Light Touch Regime, the Council has designed its own procedure. The contract opportunity is divided into five lots which reflect the Services to be delivered under the Platform. The Lots are 1. Home care; 2. Live-In Care, 3. Continuing Health Care (CHC) and Complex Care for Adults and Continuing Care (CC) and Complex Care for Children; 4. Bridging Services; and 5. Extra Care. This notice advises of the successful providers that have been added to the platform to date. Further notices will be published as new providers join the platform. There is no limit to the number of providers who can be awarded onto to the Platform insofar as the requirements of the Authority are met. To provide the ability to commission in the future, the Council may choose to add additional Lots onto the Platform during its lifetime. These lots may include Reablement, Intensive Bridging or similar described services. New and existing Platform providers can apply for any additional lots under the Platform Agreement. The Authority reserves the right to modify the platform during its operation. The Authority also reserves the right to terminate the operation of the platform at any time. Provision of a Wrap Around Service (ASC130) Sandwell Metropolitan Borough Council invites tenders for the provision of a Wrap Around Service. The Council has committed with its health partners to provide a short-term Wrap Around Service to people who require care in their own home. This commitment is to ensure that people in the community are supported in their own homes to remain independent or to support a person being discharged from hospital who may need support whilst longer term arrangements are being established. The principle of the service is to ensure people's independence is maximised and individuals are not formally entered into the care and support system unnecessarily. The aim of the service is to promote a Home First ethos and support individuals to remain in their own homes by providing Care Workers at short notice, and within an hour of contact, to step in and prevent community crisis, and provide care and support to enable an assessment of an individual's needs, for up to 72 hours. This may include offering day and/or night sits to support a person for up to 72 hours. The Wrap Around Service will enable people with or without assessed needs under the Care Act to have access to a short-term provision (up to 72 hours) to support them to continue to live independently in the community or prevent a hospital admission whilst longer term needs are being assessed. Organisations with the capability to provide a service at short notice, and within an hour of contact, for extended periods where day or night sits may be required, are invited to tender. The total cost is estimated to be £457,531.20 per year subject to referrals being received that match predictions and any potential uplift in care costs.
