Provision of a Dementia Support Service

Derby City Council (the Council) is undertaking this procurement to establish a contract for the provision of a Dementia Support Service. Derby City Council and Derby and Derbyshire Integrated Care Board (ICB) are working together to fund support for people diagnosed with, or experiencing the symptoms, of Dementia and their carers in Derby City. The aim of this service is to enable easy access to appropriate information and advice for individuals with Dementia and their carer(s), to develop a range of activity-based groups and to facilitate peer support networks for people with Dementia and their carers in all areas of the city of Derby. • To provide easily accessible information for people who are living with or have symptoms of Dementia and their carer(s). • To enable people to live well with Dementia and remain living in their own homes and communities for as long as possible. • To reduce social isolation for people with Dementia and their carers by providing opportunities to access peer and carer support in their local area. • To advise and signpost people with Dementia and their carer(s) to access support options at diagnosis and throughout the Dementia pathway. • To improve access for currently under-represented groups including different ethnic communities, young onset Dementia, people with sensory impairments and those with a learning disability. • To provide advice and practical support for people with Dementia and their carer(s) to support their independence e.g. welfare benefits support. • To increase the knowledge of carers by providing specific information to develop their confidence, resilience and ability to plan for their future care and support. • To actively engage in and promote partnership working and co-ordination between services for people with Dementia and their carer(s). • To work flexibility and innovatively to ensure on-going support as required throughout the duration of the Dementia pathway. • To ensure an open-door policy is maintained so people can access the Service at any time for further support when they need it most. North East Lincolnshire Dementia Support Service

Humber and North Yorkshire Integrated Care Board (hereinafter referred to as HNY ICB) hereby invites applications from suitably qualified and experienced providers to provide a dementia support service in North East Lincolnshire for a 3 year period. The overall aim of the service is to facilitate a formal and coordinated network of support providers to ensure people in North East Lincolnshire have timely and appropriate access to good quality information, advice, and guidance around dementia and the wider impact of dementia on the person and their carers/family including friends and connections. It also aims to facilitate access to a wide range of activities based on the person's life and interests to enable the person to remain socially connected for as long as possible, with the ambition of delaying deterioration attributable to dementia. Innovation and efficiencies in service provision for improved access and delivery of outcomes will be sought from prospective providers of the service, as will enhanced partnership working to provide an integrated service that works across health, social care, education, voluntary & community sector, and other agencies within North East Lincolnshire, providing a seamless approach. The provider will be expected to deliver a networked service based in North East Lincolnshire and throughout local communities, covering and working with all Primary Care Networks and North East Lincolnshire Health & Care Partnerships to ensure those with dementia related needs and their carers within the boundaries of North East Lincolnshire are identified, recognised and supported. Further details of the services required can be found in Annex A of the MOI. TUPE is likely to apply to the transfer of staff undertaking the current service provision.
