This notice is an intention to award the contract under the Provider Selection Regime - Most Suitable Provider process.
The Darlington Health Squad Service is based on a proven outreach-first model established in County Durham to tackle health inequalities and improve population health and wellbeing. Delivered in partnership between the NHS and voluntary sector, the Health Squad adopts a community development approach and provides an offer of holistic health assessments, delivery of health interventions, health care system navigation and advocacy. Addressing immediate health needs, longer-term engagement with healthcare when needed, and support to address the wider determinants of health.

Westminster HealthWorks

Westminster HealthWorks is a borough-wide, community-led public health initiative commissioned by Westminster City Council as part of the #2035 commitment to improve population health. The Service aims to reduce health inequalities by delivering culturally responsive, preventative health interventions that improve heart, lung, and mental health outcomes for adults aged 16 and over. Delivered through a hub-and-spoke model, HealthWorks will be led by a Lead Provider (the hub) that subcontracts with a network of at least 7 voluntary and community sector (VCS) organisations (the spokes).
