Description

Telford & Wrekin Council are looking to receive applications from Providers to join a multi-provider Framework to deliver specialist care services for Adults with Learning Disabilities, Autism, Mental Health and Acquired Brain Injury needs, in suitable locations. 
We want to work with established, experienced providers who deliver a curious, compassionate and solution focused approach central to delivering a person-centred service.
The services we intend to commission are intended for adults, but it is expected that providers, where appropriate have the correct registration to support young adults during transition and working in line with the Preparing for Adulthood (PfA) principles and support delivery of outcomes around employment and training, friendships, relationships and communities, independent living and good health.
The services will be split into 4 Lots covering increasing levels of need and complexity.

TD2320 - Family Group Conferencing

Derby City Council is undertaking this procurement to establish a contract for provision of Family Group Conferencing. The initial contract period is 1 year. The contract is estimated to commence 1st March 2026 and expire 28th February 2027. There will be an option to extend the contract for 12 months, subject to satisfactory performance and business needs. Maximum contract period is 2 years. There is a maximum budget of £90,000 per annum, a total of £90,000.00 over the initial contract term. Should the contract be extended for a further 12 months, the total contract value shall not exceed £180,000.00 over the full contract term inclusive of any extensions. The service will: • To facilitate and deliver high quality Family Group Conferences (FGC) in Derby, in accordance with the identified policies, practice standards and aims of the Family Group Conference Service. • To ensure that the service delivered is effective, of exceptional quality and value for money. • To ensure that preparation for the FGC reflects any existing formal or statutory plans in place (e.g., Child Protection Plan, Child in Need Plan). • To fully include the voice of children, young people and their families in decisions relating to the services being provided. • Ensure that the Family Plan is child focused, does not compromise the safety of children and meets the expectations of the referring agency. • To improve outcomes for families in Derby City.
