Description
The Healthy Child Programme is an integrated service for expectant parents, children, young people and their families, delivering interventions from the pre-natal stage up to the age of 19. The programme provides place-based interventions and support, led by a Specialist Community Public Health Nurse workforce. Every family is offered an evidence-based package of screening, developmental reviews, information and guidance to support parenting and healthy outcomes. The use of communitybased assets is central to the universal offer and to address inequalities. The service will build on Salford's assets and strengths, working in partnership with the full range of children's services, and where appropriate adults' services such as education; children's social care; Early Help; VCFSE partners, as well as working with NHS services for example, primary care, midwifery, paediatrics and mental health services. We're seeking an innovative and ambitious partner to deliver this essential, universal programme. By working collaboratively the Healthy Child Programme will be a key part of the Salford system ensuring every child and family receives the support they need to achieve their full potential.
