Direct Award Reintegration Services (MATRIX) - Reconnect

Description

Please note this contract has been procured and awarded by Flintshire County Council (not Denbighshire). 

This contract award has been made via a Direct Award as permitted under ESPO Framework 653F Managed Services for Temporary Agency Resources Lot 4 Statement of Work (Project Delivery) Work order is placed with Innovate (H & S C) Services Limited. 

Project title- Transition of children from residential care

Targeted intervention programmes with Flintshire to transition
children from residential care to more beneficial care settings,
including foster carers.

Programme initiation / validation of the cohort goals - Reconnect will introduce the overall Programme plan, and agree on detailed actions to achieve success, including where applicable the foster carer recruitment process. A key end goal is to use the outputs of the
children & young people assessments to confirm the overall goals and profile of the Programme which thereafter will be subject to change control

Assessment Services - the core of the Validation Services is to validate the assumptions / assessments made about the children in the Programme, by codifying need, and developing per child / young person intervention plans.
