The Leeds Electronic Market Place (EMP) shall provide citywide Short Breaks for children and young people with SEND who ordinarily reside in the Leeds boundary. All children and young people accessing the services have an Education, Health & Care Plan (EHCP) or waiting for an assessment. Children and young people accessing the service may require support with one or a combination of the following needs: personal/intimate care, medical interventions, behaviour management, high levels of anxiety, communication and interaction needs.

REOPENING of Bespoke Framework Residential Rehabilitation for Drugs and Alcohol Treatment and Recovery - Round 4 Round 4 - Framework for residential rehabilitation placements, under the Light Touch Regime (LTR) using a bespoke framework procedure. The specification outlines the minimum quality, care standard and performance management expectations for purchased services for residential rehabilitation. The framework will consist of: • Residential Rehabilitation service for drug and alcohol treatment and recovery. The framework contract commenced on 01 May 2023 for a period of 3 years to 30 April 2026 the option to extend for 1 further year. Please note the extension has been taken and will run to 30 April 2027. Under the LTR Regime the bespoke framework criteria is as follows: • Tenderers who meet the selection criteria of a minimum quality score of 40% shall be deemed to be compliant. • Round 4 of the framework is to top up the existing number of providers up to a maximum of 20. The seven (7) highest scoring tenders who meet the selection criteria will be added to the existing framework. • Providers already accepted onto the framework do not need to reapply. • If the number of providers on the Bespoke Framework falls to 15 the Council reserves the right to reopen the Framework, in line with the process detailed below: • Following a review of compliant tenderers who met the selection criteria, but were not awarded a place on the framework, the 5 highest scoring compliant tenderers shall be offered a place on the framework. Subject to no changes to CQC rating and delivery model. • If the Council does not receive a response to its offer within the timeframe identified, it shall reserve the right to withdraw the offer and offer the place on the framework to the next highest scoring complaint Tenderers. Subject to no changes to CQC rating and delivery model. • If, based on the original tender submissions, there are insufficient compliant Tenderers to meet the numbers required for the framework, the Council shall reopen the Framework to new expressions of interested by issuing an Invitation to Tender. Tenders shall be assessed using the selection and scoring criteria defined in the original tender documents. The highest scoring compliant Tenderers shall be added onto the Framework, to the maximum number of providers of 20. • Should the number of providers fall to 15 at any point in the contract period, the Council reserves the right to repeat the process as detailed above. The framework contract commenced on 01 May 2023 for a period of 3 years to 30 April 2026 with an option to extend for 1 further year. The Farmwork has been extended to 30 April 2027. Provision of Care and Support in Newton House Extra Care Housing Scheme, Newton Road, Penrith CA11 9FY

In Westmorland and Furness the term 'extra care' housing is used to describe housing developments that comprise self-contained homes with design features and support services to enable people to self-care and continue to live independently. The 'extra' in extra care housing is generally recognised to be access to care services that can: • Respond quickly to residents changing needs • Provide unplanned care as and when required in addition to planned care • Provide an emergency response. People who live in Extra Care housing have their own self-contained homes - usually with a lounge, kitchen, bedroom and bathroom and access to care staff 24 hours a day, 7 days a week. There are also communal facilities and activities available. Newton House has 54 self-contained units consisting of: a. 12 x 1 bedroom rental units b. 14 x 2 bedroom rental units c. 28 x 2 bedroom units shared ownership There is a range of Facilities: • Resident Lounge, • Restaurant • Activity rooms, • Hairdresser • Buggy store • Assisted bathing • Staff offices plus an Alarm Call System • Car park • Guest Suite • Lift • Communal gardens For many people, Extra Care housing is the ideal solution as it means that they live in a home of their own, can stay together as a couple even with different levels of need, live as part of a supportive environment, in a location situated in the heart of the local community, but with the added reassurance that help is at hand. Extra Care housing allows people to move out of houses, where they are finding it difficult to cope, into a home where they can be secure and live independently. In some situations Extra Care Housing also allows people to move out of residential care and return to independent living.

Housing Related Support - Floating Support

In 2019, Essex County Council (ECC) commissioned the Floating Support Service, which provides a broad range of short and medium- term interventions to prevent homelessness and address other housing related needs. Operating a peripatetic model, the service delivers flexible, outreach support across the County; providing targeted, cost-effective interventions to vulnerable adults and their families, who are at imminent risk of losing their accommodation, or whom may be experiencing a period of crisis. The service is aimed at people across a range of housing tenure, including those accommodated in social housing, owner-occupiers, private rented accommodation, as well as providing support to those who are homeless and require support with applying for accommodation. Essex County Council are seeking a floating service that can provide: 1. Targeted floating support to individuals and/or families presenting with complex needs including, but not limited to, threatened homelessness, substance misuse, domestic abuse, offending, mental-ill- health, neuro diversity, physical/sensory impairment, financial hardship/exclusion, care leavers and refugees with access to public funds. Floating Support encompasses a broad range of activity. 2. For the purpose of this specification, Floating Support may include any intervention or assistance that enables individuals who are vulnerable, at risk of homelessness or are experiencing a period of crisis to manage and maintain their homes across all forms of tenure. 3. More generic and early interventions to a wider range of individuals and/or families who are homeless or at risk of homelessness with less complex presenting issues, where earlier and less intensive interventions will prevent deterioration 4. The ability to flex provision to support the identified target groups to ensure opportunities to "step up" and step down" as and when appropriate. 5. Clear service discharge processes that promote a positive and sustainable outcome, but with the opportunity to re-engage with the service in the event of emergency or crisis 6. Support for those with lived experience to access peer support and volunteering opportunities (possibly in conjunction with other services) as well as the potential for Mutual Aid. Support is time limited and targeted and aimed at enabling the individual to reach a point where support is no longer required. Support will be provided in partnership with other specialist services including, but not necessarily limited to, health, social care, criminal justice and other public/voluntary sector partners. In relation to the information regarding the contracting authority within this notice, please be advised that the contracting authority is Essex County Council and any Successor Body meaning one or more entities which assumes any of the Council's functions (as applicable) as a result of Local Government Reform (LGR).
