Description
The service is required to provide support to adult carers of people aged 18 and over in Derby. The service will meet our statutory obligation to deliver information and advice to carers, is a key component of, with adult social care, the carers support process and includes a proactive support offer to reduce, delay and prevent the need for formal support for them and their cared-for in the future. The key aims and objectives of the carers support process are to: • Identify (and support self-identification of) carers to enable and encourage them to access support at the earliest opportunity. • Build carer resilience to support them to remain and be effective in their caring role for as long as possible. • Support carers to maintain their physical, mental and emotional health and wellbeing. • Take a strengths-based approach to carers support recognising what is important to the carer and the assets they have available to support them. • Understand what support the carer has and needs, and empower carers to access this, whilst ensuring it remains affordable, appropriate and proportionate. • Demonstrate that carers are recognised and valued for what they do, and that they feel heard. Support carers to consider and plan for what may happen in both the foreseeable, longer term and sudden, shorter-term changes in circumstances. • Provide support to carers to self-serve at different points in their caring journey including when caring begins, becomes more intense and changes or comes to an end. • Improve carer inclusion and connectivity in carer, local and wider communities. • Develop relationships with and improve carer access to key partner and community organisations. • Fulfil the Council's obligations under the Care Act to provide information, advice and assessments to carers. • Apply effective and efficient allocation of funds and resources to minimise cost for the best outcomes. IMPORTANT Potential bidders are asked to be aware of Local Government Reorganisation in Derby and Derbyshire on contracts and service delivery in future. This may require changes to service volumes, geographical boundaries for service delivery and available budgets.
