Description

Bath & North East Somerset wishes to establish a single provider Contract for the provision of the ASC Sensory Impairment Support Services . The procurement is run in accordance with the Procurement Act 2023, Light Touch Regime (LTR) below-threshold, one stage tender.
The Sensory Impairment Support Service will provide support to people who live in B&NES or are registered with a GP in B&NES who have a hearing and or visual impairment. The service will offer Information and Advice, access to assistive equipment and facilitate social activities and opportunities. The service will take a person-centred approach and provide flexible and responsive service, promoting individuals' ability to maintain their independence and build connections within their communities.
The service will provide comprehensive information, advice, support, and representation to individuals living with hearing and sight loss in Bath and North East Somerset. This includes offering assistive equipment, facilitating social activities, and providing community outreach support. Additionally, the service will offer guidance on communication needs and signpost individuals to relevant courses as appropriate.

Provision of a Supported Living Service within Supported Accommodation

NAC and The North Ayrshire Health and Social Care Partnership wish to work in partnership with providers to deliver a high-quality care and support service within two Supported Accommodation Housing sites in North Ayrshire. Both sites are owned by North Ayrshire Council. The scope of the service will encompass support to manage and maintain a tenancy providing personalised supports to individuals; and to achieve individual outcomes aligned to personal support plans. Support will include:- -Personal Care -Housing Support -Other practical support required to maintain their tenancy The contract will be let on The North Ayrshire Health and Social Care Partnership Standard Terms and Conditions. The total budget for this requirement will be 15,296,039.20 GBP. The contract will be in place for a total of 10 years, initially for a 4-year period, with an option to extend by 72 months. The anticipated contract start date will be 1 April 2026. Lot 1 budget - 7,507,000.80 GBP. Lot 2 budget - 7,789,038.40 GBP. The applicable regulations will be The Public Contracts (Scotland) Regulations 2015 Light touch regime for services.

Bath and North East Somerset ASC Support at Home Services

Bath & North East Somerset wishes to establish a single provider Contract for the provision of ASC Support at Home Services. The procurement is run in accordance with the Procurement Act 2023, Light Touch Regime (LTR) above-threshold, one stage tender. The Support at Home Service will help people remain living at home, gain in confidence, and provide short term practical help after hospital discharge or following community or out-patient medical care to prevent admission to hospital. The service will deliver high quality support to individuals, free at the point of delivery, accepting referrals from health and care services, and approaches by the public. The service will provide an easily accessible front door via the Community Wellbeing Hub referral process and meet the needs of individuals across the geographical area of B&NES. It will actively engage with health, care and other third sector providers, promoting its services, ensuring that providers are aware of services offered. The service provider will be an active member of the Community Wellbeing Hub and similar local partnerships, to support cross working, referrals and information sharing. The service will work in parallel to and in collaboration with other available services currently forming part of the discharge support pathway, which may be reviewed in future, including CWH Hospital Connectors and Help Pack provision.

It provides care-experienced young people with opportunities to voice their opinions, influence services and policies, and improve their care experiences through regular meetings and events.
