Description

Newham Council is seeking a provider to deliver infrastructure services for the Voluntary, Community and Faith Sector.  The Council would particularly welcome a progressive partnership or consortium of providers with a clearly identified lead partner and a group of delivery partners. The contract will be for a period of two years.
The Voluntary, Community and Faith Sector in Newham is extremely diverse, ranging from larger registered charities and national organisations (such as Age Concern and Citizen's Advice Newham) to smaller community based groups often led by volunteers such as 'friends of' parks groups.
The aim of this contract is to support the achievement of a sustainable, confident and resourceful Voluntary, Community and Faith Sector that meets the current and future needs and aspirations of the sector and borough.

Aberdeen City Health & Social Care partnership required a provider to develop and deliver a Discharge to Assess service. The provider who would develop a stand alone team who would work alongside hospital staff to identify individuals who are medically fit to be discharged but require a period of recovery, assessment and enablement in their own home environment. The level of need of these individuals would normally indicate that they would require a care home environment. Following a targeted period of support at home, including overnight support where required, the individual’s need for ongoing support will be assessed and commissioned from existing providers if required.
