Description

Thurrock Council is seeking a Provider to deliver the following services:
•	Homeless Hostel Management and Support - Housing management and support services for residents in a Thurrock-based homeless hostel.
•	Floating Support - Outreach support for individuals at risk of eviction to help maintain their housing.
•	Housing First - Long-term housing and wraparound support for individuals with complex needs and histories of homelessness.
•	Rough Sleepers Outreach - Identification and engagement of rough sleepers, with pathways to accommodation and support.
The Provider will deliver six key areas of intervention as part of the Service. The Service outcomes will be: 
•	Tenancy Sustainment 
•	Arrears, Debt, and Financial Management
•	Meaningful Occupation
•	Health and Wellbeing
•	Social Inclusion
•	Resettlement

New Era - Supported Living Services for Young People

Term of Contract: Seven years with the option to extend for three years in annual increments The Service to be provided Wrexham County Borough Council wish to commission and work with a suitably experienced organisation to provide support to young people age 16+, with complex needs, within Wrexham. Young People will be supported within an existing four bedroomed, shared house in Wrexham city centre. Support will also be required for a maximum of eight young people on an outreach basis, also within the city centre. This support is provided in partnership alongside a Registered Social Landlord. There are currently plans in place for an additional two properties, providing the same type and level of service, to be included within this contract. It is expected that the provider will provide the same level of services for all three properties as well as the outreach service. It is anticipated that the second property will be ready by March 2026. The third property is still in the early stages of planning and the start date is unknown at this time. These two additional properties will not be within the city centre, but elsewhere within Wrexham County Borough. The Service aims to provide support to young people for a temporary period, assisting them to progress to independence. The length of time the young person will be supported will be determined by their individual needs and progress towards their outcomes as part of their support planning. The aim is to progress young people to move on within two years (with flexibility to extend this period), dependent on their individual circumstances. Young people will be supported to gain, re-learn or maintain life skills in order to carry out their own day to day tasks necessary for independent living. TUPE may be applicable as there is an incumbent provider delivering the service at present.
