The Tier 3 service across Norfolk and Waveney will offer the opportunity for service users who meet the qualifying criteria to access comprehensive support via a weight management programme tailored to their needs including reasonable adjustments in line with NICE Guidance. <br/><br/>The Tier 3 weight management service will incorporate both standard services and enhanced services for those service users with more complex health needs (co-morbidities) either independently or in partnership with other providers.<br/><br/>The service aims to:<br/><br/>•	Encourage long term behaviour change by offering a programme which promotes physical activity; promotes healthy eating and supports the psychological barriers to unhealthy relationships with food; supports and recognises the relationship between mental health and obesity and offers a system throughout the programme which will support the psychological needs of everyone who attends.<br/>•	Reduce the number of people living with obesity in Norfolk & Waveney, thereby improving long-term health, reducing the burden of disease, and decreasing the need for social care, while also preventing or reducing the incidence and healthcare activity related to obesity-related illnesses.<br/>•	Function as part of a seamless care pathway for adults with obesity; to proactively work in partnership with services provided at tiers 1, 2 and 4, as well as other existing services supporting behaviour change<br/>•	Undertake comprehensive stakeholder engagement to inform practice, referral confidence and workforce development.<br/>•	Work with Norfolk & Waveney ICB using Population Health Management (PHM) data management systems to assist in the identification of service users with agreed BMI and clinical thresholds to ensure the people with the highest risk can be identified and supported to access help, where needed.  This information can be produced at PCN / GP Practice level to risk stratify and prioritise patients, highlighting areas of highest need or clinical complexity.<br/><br/>The Contracting Authority will be using an eTendering system for this competitive process. Harrogate Court Extra Care Scheme

The City of Doncaster Council is seeking a suitably qualified and experienced provider to deliver care and support to tenants of Harrogate Court along with housing related and general wellbeing support and a community hub, providing a high quality service at all times. Care and support packages will be delivered in line with assessed need. It will be a 24/7 service including first response. The contract will be for a period of five years with the option to extend for a further five 12 month periods. Harrogate Court is a 50-bed apartment extra care scheme, for People aged 55 and over. The scheme has 21 one-bedroom apartments and 29 two-bedroom apartments. All apartments are for rent. (There is also a communal lounge, laundry room, hairdresser, restaurant , shop and guest room. It is a requirement of this contract that you hold a CQC rating of good or above from the start and for the duration of this contract. Harrogate Court is not currently registered with the Care Quality Commission (CQC), on contract award the successful provider must commence the CQC registration process. Until the service and manager are both registered with CQC, the successful provider must ensure that the service is overseen by a registered manager or CQC nominated individual, to provide consistent support to the manager. The nominated individual or registered manager designated must currently be responsible for a care service which is currently rated as ‘Good ‘or above with CQC.
