Description

The Norfolk and Waveney NHS Integrated Care Board and its partner stakeholders is seeking a Partnership Development and provision of a Risk Stratification / Population Health Management (PHM) Services Digital Solution to provide bespoke data analysis, technical solutions and digital platforms to underpin their PHM strategy and support the prioritisation and delivery of interventions.

N&W ICB's overall Population Health Management strategy requires analysis at various levels and enable access to and use of PHM information at all levels, across NHS and wider partners within the Integrated Care System (ICS) to support a strategically aligned approach to Population Health.

The PHM digital solution being sought must be secure, complying with all Information Governance requirements.

Data Analytics Solution

Over the past 5 years the London Borough of Barking & Dagenham (the Council) has implemented an innovative data platform called OneView. At its core, the platform loads data extracted from other Council systems and matches it at the household level to create a 'golden record' of resident data. Using this platform, we have matched data from five key service areas (Revenues & Benefits, Adults Social Care, Children's Social Care, Housing and Education) to enable a 'single view' of households in our borough. Notably, OneView can receive and process both structured and unstructured data, using natural language processing to read and interpret case notes. Building off this core infrastructure we have developed a range of tools that enable the Council to work efficiently, and deliver better outcomes for residents. DAERA requires a Supplier to provide the administration, delivery, licencing, support and maintenance of a Carbon Calculator which will provide farm businesses in Northern Ireland with their Whole Farm-level Carbon Footprint, Enterprise-level Carbon Footprint and Product-level Carbon Footprint. Training will be provided to assist farmers to use the information received to take appropriate action to reduce net GHG emissions on their farm. The Carbon Calculator procured for this project will be used on all Northern Ireland farms, and requires 28,400 farm carbon footprints to be completed over the next four financial years 2026/27 – 2029/30.
