Description

NHS Frimley ICB sought to commission a digital software application tool which supports the function of Social Prescribing across 4 Places -Bracknell Forest, North East Hampshire and Farnham, Surrey Heath and The Royal Borough of Windsor and Maidenhead, as well as secondary care partners (including adult social care, outpatients, and voluntary sector community connectors).

The purpose and aim of this procurement was to source a software solution that will connect participants to local services that can support their journey back into or to remain in work, effectively reducing pressures on Primary Care Services through the repetition of issuing fit notes and avoidable GP appointments.

The total value of the initial 1 year contract term is £122,285.58 (exc VAT). The contract includes an optional extension of one period of up to 12 months. The total value of the optional 12-month extension period is £122,285.58 (exc VAT) plus an increase by the greater of: (i) the percentage increase in the Retail Prices Index (all items) published by the UK Office for National Statistics in the preceding 12-month period; or (ii) 2% (two per cent.).

The initial 1-year contract period commenced on the 1st May 2025.
