The service provides a health outreach model with a multi-disciplinary approach for single homeless people with complex needs, including ex-rough sleepers.

Southend-on-Sea City Council (the 'Council') is seeking to establish a multi-lot framework to provide Care at Home of which is described in the Tender Specification (Section B). Southend City Council defines "Care at Home" as comprising three essential services: 1. Stockport Homes (SHL) delivers a wide range of services within a variety of different settings to customers. We are seeking to appoint a competent and professional Contractor to deliver counselling sessions to customers who are in need, in order to help transform lives. It is proposed that this service will commence on 1st September 2025 for an initial two-year period, with the opportunity to extend for one year upon satisfactory performance and sole discretion of SHL.
