The Camden Adult Pathway Partnership (CAPP) is a small clinical team that provides nurse-led outreach to 16 supported accommodation sites across Camden, known collectively as the Adult Pathway. The service provides a health outreach model with a multi-disciplinary approach for single homeless people with complex needs, including ex-rough sleepers.

Southend-on-Sea City Council (the 'Council') is seeking to establish a multi-lot framework to provide Care at Home of which is described in the Tender Specification (Section B). Southend City Council defines "Care at Home" as comprising three essential services: 1. Homecare: Personalised assistance provided to individuals in their own homes, including help with daily tasks, personal hygiene, and medication management. 2. Estimated total contract value : £110,000,000 Southend City Council (SCC) are looking to establish a Care at Home Framework in which any statutory funded organisation can utilise.
