Description

The service will provide effective management, co-ordination and signposting of High Intensity Users of the local Emergency Department (ED) at Scunthorpe General Hospital (North Lincolnshire). The service will reduce the activity High Intensity Users have on GP Practices and will establish, utilise and coordinate multi-agency and existing professional services to negotiate a reduction in 999 calls. The service aims to demonstrate a reduced workload on unscheduled care services and the wider health economy resulting from reduced 999 calls which otherwise would have attended ED and result in an admission. The service will safely manage and co-ordinate the chaotic and demanding nature of the patient group through the use of multi-agency support and the volunteer sector.

Flintshire Dementia Community Engagement Service / Gwasanaeth Ymgysylltu Cymunedol Dementia Sir y Fflint

Flintshire County Council would like to appoint a trusted service provider to deliver a Dementia Community Engagement Service. The commissioned service will implement a dedicated post or small team to engage with the community of Flintshire, providing information and support relating to dementia. This service will lead the development of dementia friendly communities and information sharing networks to increase community participation and public awareness. This engagement service will complement statutory health and social care services, and commissioned services including the memory support pathway, and dementia unpaid carers support service. This service will be funded by Welsh Government Regional Integration Funding (RIF) and will replace an existing grant funded project that delivers similar service outcomes. There will be a requirement for Transfer of Undertakings Protection of Employment (TUPE) impact to be managed between the existing and commissioned providers. The initial contract will be for eighteen months with an option to extend for an additional two years subject to the continuation of Welsh Government funding. Primary Care Domestic Violence and Abuse Service - Birmingham and Solihull ICB

NHS Arden and Greater East Midlands CSU on behalf of NHS Birmingham and Solihull Integrated Care Board (BSol ICB) are seeking suitably qualified and experienced organisations to express an interest in providing a Primary Care Domestic Violence and Abuse Service. The current IRIS provision is centred on the provision of services to women. In addition, not all areas of the Birmingham and Solihull footprint are currently covered. This has resulted in the creation of inequalities in terms of the service which need to be addressed going forward. BSol ICB intend to commission a Primary Care Domestic Violence and Abuse Service that is open to all victims or potential victims (both female and male) of domestic violence and abuse (DVA) across the BSol ICB footprint that are aged 16 and over. The new service will therefore need to: - Ensure all geographies across the Birmingham and Solihull footprint have equal service provision - Ensure all citizens have equality of access, enabling men to access the service as well as women - Ensure that any required referral pathways are well thought through and do not introduce potential barriers to access whether these are perceived or real. Patients aged under 16 years witnessing or experiencing DVA should be managed by the practice in accordance with BSol ICB Safeguarding Children procedures. Patients who have disclosed past or present domestic abuse will be referred by participating general practice staff. In some circumstances they may also refer patients where domestic abuse is suspected. Provision of support at Oasis House

West Northamptonshire Council are undertaking a competitive procurement process to commission a Support Provider to continue to deliver specialist support to individuals with multiple and complex needs with a history of rough sleeping or at risk of rough sleeping, who are accommodated at Oasis House, Northampton. The property is a fully occupied building within the commissioned supported accommodation pathway providing 59 residential units made up of 48 one-bed studio flats, and an additional 11 one-bedroom units with ensuites as well as ancillary common areas. The scheme will provide a 'preventative' service and work in partnership with WNC and its partners to accommodate people with immediate housing and support needs, preventing rough sleeping, and assisting rough sleepers in transitioning to independent living. It will also offer off-the-street rapid response beds for an initial assessment period within en-suite rooms to identify the housing and support needs of service users based on their assessed needs and risks. The goal is to minimise returns to homelessness, promote independence, prevent social exclusion, support individuals with substance misuse issues, and operate within a psychologically informed environment.
