Description

Provide direct primary care health service provision for vulnerable people with complex health needs to reduce pressures on other urgent care services, such as Urgent Treatment Centres and Emergency Departments, GP Out of Hours and NHS111. The provision of screening, immunisation and vaccination within the facility reduces the risk of spreading disease/s to other people. The mental health and wellbeing of the residents is also supported to avoid impact on other service providers.

Estimated Annual Contract Value: £453,232. Contract Length: duration of 3 years with the option to extend for up to a further 2 years. <br/><br/>The service commencement dates for all three contracts is 1st April 2026.<br/><br/>Please note the deadline for responses to the Competitive Process is 12:00PM (Mid-day) on 12th September 2025.<br/><br/>Note - the Estimated Total Value in section II.1.5 Estimated total value is based on the annual contract value of all 3 lots due to Lot 1 being in perpetuity.
