Help develop a framework for measuring the impact of project interventions using HealtheIntent data that the analyst s in the team and project leads can use in a consistent way.

Train the PHM team and other system colleagues working in PHM (this could be SEL BI, health equity fellows, those working in the office for data analytics) in a sustainable way to be able to evaluate projects confidently and correctly using the above framework.

Provide an understanding of the time, skills and resources needed within the PHM team to provide evaluations on individual projects, with appropriate visualizations on an ongoing basis.

Lead the full evaluation of 2 4 of the most promising interventions/projects by helping to pull together what could be a huge amount of data into a meaningful and accurate analysis and provide a financial economic evaluation of the interventions and projects.

Being able to present the benefit of the HealtheIntent platform what other work we could and couldn't have done without it, so none intervention trackable projects. This also means giving a view on the technical aspects of the platform.

Provide a comprehensive demonstration of the cost benefits of employing a Population Health team, including a detailed evaluation of the costs associated with the team and IT platform, compared to the anticipated savings to the system using the projects listed and opportunities for future projects.

A final report that gives an overview of all the above 4 aspects.
