Description

NHS South West London Integrated Care Board (ICB) is developing an Integrated Care Strategy to set the strategic direction for health and care services across the whole geographic area of the Integrated Care System of South West London. Central to the strategy is a focus on activity which keeps care close to home and improve self referral routes. The Home from Hospital Service in Sutton supports care closer to home and supports Sutton residents to be empowered to connect with onward services which could benefit them.
The Contract to be offered to the Preferred Bidder at the end of this Procurement will be for an initial duration of three years with an extension of two years. The intended duration of the contract therefore is from 1st April 2025 until 31st March 2028 with option for extension until 31 March 2030, subject to any restrictions placed nationally by NHS England or the Department of Health and Social Care.

The contract value for the initial three year period is £381,000 with the maximum total contract value (including the two year extension) of £635,000.  The annual value is estimated at £127,000 per annum.

KCC is seeking a suitable organisation to propose and deliver a programme of non-clinical Level 1 Universal Parent Infant Relationship Interventions, to test and pilot new ideas. Level 1 Parent Infant Relationship Intervention will comprise of the following elements. •Creative group sessions – Interactive for parents/families with their baby/Infant •Facilitated group discussion – to include peer and mutual support •One to One support and feedback to parents. As part of this KCC would like to see creative group sessions that offer early support for those families with a newborn/infant up to 6 months. Families are defined as mums, dads, new parents, co parents, partners and carers which include family members who play a vital role in supporting parent infant relationships and the family's mental health.
