Buckinghamshire Council is seeking a single provider to deliver care and support into four residential establishments in Buckinghamshire through a block contract
1.	The Croft Care Home is a 60-bed residential Establishment. Included within the block contract are 25 residential and 27 Residential Dementia beds. The remaining 8 beds are available for the Provider to use at their discretion.
2.	The Chestnuts Care Home is a 64-bed residential Establishment. Included within the block are 16 residential and 27 residential dementia beds and 12 Extra care beds. Willowmead Court is a 12-unit extra care facility attached to the Care Home. The remaining 21 beds are available for the Provider to use at their discretion.
3.	Hazlemere Lodge care home is a 64-bed residential Establishment with nursing. Included within the block Agreement are 32 Nursing beds, 16 residential beds and 12 Extra Care beds. St James Court is a 12 unit extra care facility attached to the care home. The remaining 16 beds are available for the Provider to use at their discretion.
4.	Swan House care home is a 32-bed residential Establishment. Included within the block are 10 residential and 8 residential dementia beds and 12 extra care beds. Swan Court is the 12-unit Extra Care facility attached to the Care Home. The remaining 14 beds are available for the Provider to use at their discretion.

GMCA 1386 - Greater Manchester Housing First and Rough Sleepers Accommodation Programme

In 2019 the Ministry of Housing Communities and Local Government (MHCLG) commissioned programme of three pilots with Greater Manchester Combined Authority, Liverpool City Region and West Midlands Combined to pilot the large-scale delivery of Housing First in England. The service is for people who have multiple and complex needs who have experienced long term recurrent periods of homelessness and exclusion from services leading to them having difficulties in finding and sustaining suitable accommodation. The pilot has now been delivered for 6 years supporting 568 people to date across the 10 Greater Manchester Local Authorities. GMCA were successful in competitive bidding processes to secure Rough Sleeper Accommodation Programme (RSAP) properties across Greater Manchester, there will be a maximum of 58 properties in this contract. This tender is to procure a Housing First and RSAP services for a further 12 months (with option to extend for a further 3x12 months) to continue to support those individuals on the programmes including housing those that are yet to secure a permanent offer of accommodation. The Provision of Care and Support Contracts at Six Extra Care Schemes

Brent Council is seeking tenders for the Provision of Care and Support Contracts at Six Extra Care Schemes for a period of three (3) years with the option to extend by a maximum of two (2) years on a one plus one basis (3+1+1). Providers must be able to meet the needs of a mixed group of residents from the community with a range of age groups from 55 years + and a range of needs including adults with physical needs, sensory, mental health (including Dementia) and learning disabilities as well as those with complex needs and behaviours that challenge. This contract opportunity is divided into the following Lots: • Lot 1 Beechwood, Harrod & Tulsi • Lot 2 Rosemary, Newcroft & Willow The contracts will be based on a core and flexi model with only the core element being the “contracted amount”. The core service is designed to meet unplanned and emergency care needs and night support. The flexi hours will be based on service user’s assessed needs and support hours will fluctuate as individuals’ needs change. Residents will be able to choose another care provider to deliver their assessed hours, or arrange their own care through a Direct Payment, but the successful provider will deliver the core hours in the service.
