Description

The Provider will deliver a health, wellbeing, lifestyle and behaviour questionnaire for Dudley's children and young people on behalf of Dudley Council, to be carried out biennially in mainstream primary and secondary education settings, colleges, and in settings for children with additional needs by pupils in year 5, 6, 8, 10, 12 and 13.  The provider will be required to design an inclusive, innovative health, wellbeing, lifestyle and behaviour questionnaire for Dudley's children and young people, and provide the questionnaire in both an online and paper format. The questionnaire will focus on general health; healthy eating and physical activity; drugs alcohol and smoking; mental health and wellbeing; personal safety; bullying behaviours; and relationships and sexual health. The provider will produce easy to follow instructions on the administration of the questionnaire for each type of participating setting. The provider will collate and analyse results for all participating settings, on an individual and a whole borough level. Where possible, provide data comparisons to neighbouring areas or statistical neighbours or nationally. The provider will provide accessible, comprehensive reports to the commissioner and individual participating education settings and deliver results briefings to partners. The provider will abide by all other requirements as detailed in the service specification.
