Flexible Framework for the Provision of Supported Living and Community Based Support Services for Adults with Social Care Needs

Renfrewshire Council (the Council) is seeking to establish a multi–Provider Flexible Framework Agreement (Flexible Framework) for the Provision of Supported Living and Community Based Support Services for Adults with Social Care Needs. The Services will be for Adults with support needs relating to Mental  Health, Learning Disabilities, Autism, Physical Disabilities, complex needs or other care and support inclusive of personal care, housing support, non-personal care, emotional and practical support. There may be the occasional requirement for a Service to be arranged for a child. The Council will utilise the Flexible Framework for SDS Option 2 & Option 3.
It is anticipated that the Flexible Framework will be for 4 years with the option to extend for up to 12 months on 3 occasions, subject to satisfactory operation and performance. It is anticipated that the Flexible Framework will start on the 1st November 2024 and will expire on the 31st October 2028. Should all extension options be taken, the end date will be the 31st October 2031. In the event that the Flexible Framework does not start on 1st November 2024, the Flexible Framework will start on the Commencement Date as stated in the Letter of Acceptance. The length of any Individual Service Contract (ISC) made under this Flexible Framework will be stated in the relevant ISC. Please note an ISC may expire after the Flexible Framework End Date.
Tenderers should note there is no guaranteed level of spend for any ISC awarded under the Flexible Framework; there is also no guarantee of payment being made to a Provider solely by appointment to the Flexible Framework.
A flexible approach has been designed which will allow New Entrants to apply to be on the Flexible Framework.

NHS South West London Integrated Care Board (ICB) (Croydon Place) is seeking to commission Long-Term Conditions (LTC) Community Outreach, Expert Patient Programme (EPP), LTC Self-Management and Pentathlon Healthy Lifestyle Programme Services. Since 2020, the LTC Community Outreach and Expert Patients Programme has been focussing on prevention, early identification and improved management of LTCs in Croydon with the aim to raise awareness with people who do not regularly access GPs or local NHS services. With the population in Croydon with two or more long term conditions at 23% and set to increase, a collaborative approach across all long-term conditions is key to manage, support and prevent increases in the number of Long-Term Conditions and the costs associated with that to health and the individual. The aims of the LTC community outreach and EPP programme are to deliver cost-effective, culturally competent, and preventative community health interventions that address the health inequalities in Black, Asian and Minority Ethnic (BAME) communities which constitute circa 50% of SWL ICB's core20 communities, particularly in deprived and underserved wards such as Thornton Heath and New Addington. In particular the LTC community out-reach health and wellbeing checks, Pentathlon healthy lifestyle courses and Expert Patient Programme (EPP) LTC self-management course are anticipated to: • Support raising grass roots awareness of LTC risk and risk factors and support people to adopt healthier lifestyles to reduce the risks of LTC, support early identification and diagnosis and self-management in key Croydon communities. • Empower patients with LTCs to feel more confident in managing their own condition and keep them active and engaged in their lives. • Tackle health inequalities and increase uptake from hard-to-reach groups. The Contract is expected to commence in April 2026 for an initial period of 2 years. The contract may be extended for an additional period of 1 year at the sole discretion of the Authority.
