An integrated solution for the management of patients with long term conditions or episodic events in their place of residence and other locations remote from Primary care.  The service is applicable to any patients in the community with an ongoing requirement to monitor a medical condition (whether long-term or episodic).  The software solution will provide the means for the collection and analysis of essential patient related data, which allows for effective management through efficient interaction between clinicians and patients.
