Procurement for Intravenous Therapy and Bisphosphonates Service on behalf of Greater Manchester Integrated Care Board

The aim of the IV therapy and Bisphosphonates service is to enable Stockport patients aged 18 years and over who are medically stable to safely receive intravenous therapy in a community health care setting or the patient's own home for selected frail and housebound patients, thereby reducing avoidable hospital admissions and facilitating early hospital discharge. The community IV service is predominantly to avoid hospital admissions (step-up) for patients requiring IV antibiotic once daily treatment (OD) or twice daily (BD) for a short duration (average 3 days, maximum 15 days). The service will also accept a number of referrals for step down patients from hospital. Where clinically appropriate the service will empower patients to self-administer. The key objectives of this service are: 1. To provide high quality care closer to home to meet patient's needs. 2. To avoid hospital admissions. 3. To support early hospital discharge. 4. To provide timely access to assessment, treatment and care which is safe and effective to manage patients requiring IV therapy in the community. 5. Following clinical assessment and where capacity permits to offer IV hydration therapy for the treatment of dehydration. A management plan will be in place to help avoid hospital admission. 6. To support the Stockport sexual health team in Out of Hours and bank holidays with patient who require daily IM injections of Procaine. New Chance Service

New Chance is an intervention for women over 18 years, intended to divert them away from entering further into the criminal justice system. West Midlands Police are the single referring agency into the service and make referrals primarily through Out of Court Resolutions but may also make voluntary referrals where appropriate. The New Chance service aims to: - Divert women from the formal criminal justice process by providing an effective community-based alternative to prosecution or conviction and potential short custodial sentences.  - Address underlying and root issues contributing to offending behaviour, including but not limited to substance use, mental health, housing instability, domestic abuse and socio-economic factors, through support tailored to an individual woman’s needs.   - Provide trauma- and gender-responsive support that recognises and meets the specific needs of women in the criminal justice system, considering their individual experiences.  - Improve community safety by promoting rehabilitation, reducing re-offending rates and improving women’s outcomes e.g. education, training and employment and family relationships. Achieved through empowering women to lead safe and healthy lives.   - Work collaboratively with statutory and non-statutory community-based partners to ensure a coordinated multi-disciplinary approach that leverages local knowledge and resources to achieve the best outcomes for women.
