Provision of Continuing Healthcare Fast Track Services for Nursing Home Beds

The purpose of this Service is to deliver care within a Residential Care Home with Nursing (referred to from this point as a Nursing Home) for those patients who have been assessed as eligible for NHS Fast Track funded care, have expressed a preference for care in a Nursing Home and whose needs can be safely and appropriately managed in a Nursing Home. The Service is commissioned to provide dignity and assurance to people and their families when they are entering their final weeks of life and is expected to be safe, responsive and efficient. This includes enabling patients to achieve the best possible quality of life whatever their disease or disability, gender, ethnic background, or sexual orientation. This will be achieved through the application of a holistic person-centred approach to care which is integrated seamlessly with all other health services, especially GP primary medical services and specialist palliative care services. The aims of this Service are: • To deliver a responsive, caring and person-centred service for patients and their loved ones in the last weeks of life. • To facilitate prompt commencement of care in a Nursing Home, for patients identified as in the end stages of life under the NHS Continuing Healthcare Fast Track pathway. • To commission end of life care which is delivered by staff with enhanced levels of training and competencies in relation to end of life care. • To prevent inappropriate or avoidable admissions to hospital for end-of-life care. Please note that all Lots are restricted to a maximum of 2 beds per nursing home, except for Lot 2 (which is 3 beds). You must be able to deliver the maximum number of beds stated for the Lot you are bidding for. Bids received with less than the maximum or more than the maximum stated Beds will be excluded. You may bid for any Lot, but will only be awarded a single Lot per nursing home. • Lot 1- West Norfolk 2 x nursing beds circa £114,000 per annum ( VAT is not chargeable for this service) • Lot 2 – North Norfolk 3 x nursing beds circa £171,600 per annum (VAT is not chargeable for this service) • Lot 3 – Norwich 2 x nursing beds circa £114,000 per annum (VAT is not chargeable for this service • Lot 4 – Norwich 2 x nursing beds circa £114,000 per annum ( VAT is not chargeable for this service) • Lot 5 – Norwich 2 x nursing beds circa £114,000 per annum ( VAT is not chargeable for this service) • Lot 6 – Norwich 2 x nursing beds circa £114,000 per annum (VAT is not chargeable for this service) • Lot 7 – South Norfolk 2 x nursing beds circa £114,000 per annum (VAT is not chargeable for this service) The contract is for a period of 2 years with the option to extend for up to an additional 2 years. Prior Notice Published: UK-0339 The East of England NHS Collaborative Procurement Hub intends to establish a multi-lot framework agreement to support the commissioning of services that address health inequalities and the wider determinants of health. The framework will offer a compliant, structured route for NHS organisations, Integrated Care Boards (ICBs), local authorities, and other public sector bodies to procure community-based services. It is designed to strengthen collaboration with VCSEs, SMEs, and specialist providers, with the aim of improving early access to support and reducing pressure on acute services. The framework focuses on three core themes: Voice & Influence: Enabling people and communities to shape services, exercise choice, and be active participants in decision-making. Community-Led Services for Social Impact: Supporting services led by local organisations and people, with a focus on addressing needs identified by the community. Prevention & Cessation: Commissioning services that focus on prevention, early intervention, and supporting people to make positive changes, reducing demand for crisis or acute care. Services may include, but are not limited to, support with mental health, housing, employment, financial inclusion, domestic abuse, and wider prevention services. The framework will be open to all eligible providers across the UK and will operate as an open framework under the Procurement Act 2023.
