Description

Doncaster Council are looking to increase the reablement capacity within Domiciliary Care to ensure that people leaving hospital who require reablement support are able to access services in a timely manner. This additional capacity will work alongside the STEPS service currently provided in house by Doncaster Council.
 
Access to timely social care services is critical to ensure that flow continues through the systems and individuals are able to access appropriate services without delay.
 
On that basis, it is proposed that Doncaster Council seeks to engage a registered domiciliary care provider to deliver a block contract for domiciliary care services to individuals leaving hospital or requiring step up care to avoid short term stays in residential care. This will provide additional capacity in the City to people needing immediate support either to avoid an admission to hospital or to support them to go home after a hospital stay.

Electronic Interventions will need to be accessible by users using any 'smart device' (smart phone, tablet, computer) and be available for users to work their way through a structured programme in their own time and at their own pace.
