Description

Swansea Bay University Health Board (SBUHB) is looking to return the Cymmer/Cwmavon General Practice, which is currently directly managed by the Health Board, to independent contractor status, with the intention of awarding a permanent standard NHS General Medical Services (GMS) contract for the provision of primary health care. If you are interested in taking forward this exciting challenge, we would like to hear from you.  The model of service expected is set out in the NHS (Wales) standard model GMS contract.
The Cymmer/Cwmavon Health Centre is based across two sites. The primary site is located in the large village of Cwmavon which is approximately 2 miles from the industrial town of Port Talbot.  There is also a site located in the small village of Cymmer which is 7 miles from Cwmavon. Both premises are located within the Afan Valley, which is a green and mountainous area of South Wales.  To the west lies the city of Swansea and some thirty miles east is Cardiff.  There are good road links from both cities and train links to the town of Port Talbot.
The practice delivers a model of care, which is supported by a multi-disciplinary team that includes 5 Salaried GPs (4.3 FTE), 1 Physician Associate (1WTE), 1 Advanced Nurse Practitioner (1WTE), 1 Advanced Clinical Practitioner (1 WTE), 1 Pharmacists (0.8 FTE), 6 Nurses (3.5 FTE) and 3 Health Care Support Workers (1.9 FTE).
The practice provides all essential and additional services and a range of enhances services.  The current practice list size is 7859.
The practice is supported by the wider community teams, which are focused on GP Practices and include Health Visiting and District Nursing.
Practices do not have Out of Hours commitments as this service is provided by SBUHB.

KCC is seeking a suitable organisation to propose and deliver a programme of non-clinical Level 1 Universal Parent Infant Relationship Interventions, to test and pilot new ideas. As part of this KCC would like to see creative group sessions that offer early support for those families with a newborn/infant up to 6 months. Families are defined as mums, dads, new parents, co parents, partners and carers which include family members who play a vital role in supporting parent infant relationships and the family's mental health.
