Description

The Care Co-ordination Hub provides a senior level, multi-disciplinary, clinical consultation service, including the ability to prescribe, which assesses patients' needs and navigates them to the right service, first time. Referrals are received from both the South Western Ambulance Service Trust (SWAST) Stack of 999 calls and from Paramedics on scene, calling for advice and to explore alternatives pathways to the Emergency Department. This provides an opportunity for shared clinical assessment and decision making between the Hub and the referrer, with the potential for the call to be closed or referred onto existing services in the community if required. There is also an opportunity to link in with, or refer onto, the three Locality Spokes across Devon for patients who have more complex needs, or for an ambulance to be dispatched if required. The staffing model is made up of Advanced Care Practitioners and General Practitioners, all of whom are independent prescribers, working closely with SWAST Specialist Paramedics.

Provision of Hypertension and Lipids Management Services

NHS Norfolk and Waveney Integrated Care Board is seeking a procure a Hypertension and Lipids Optimisation service. The successful provider staff will be expected to work with the ICB Cardiovascular disease team, the N&W Protect NoW team, and General Practice to improve clinical effectiveness through case finding and management of patients.<br/><br/>NHS Norfolk and Waveney Integrated Care Board is seeking to launch a large-scale CVD Prevention project using a Population Health Management approach to case finding and intervention. The project will focus on hypertension and lipid management, with patients being identified and subsequently risk-stratified using the ICB software.<br/><br/>The ABCDE program aims to improve the care of patients at risk of Cardiovascular events through case finding and improved management of high-risk individuals. This project is considered a system priority to see our population lead healthier, longer lives, whilst simultaneously reducing pressures on the health system through early detection and management.<br/><br/>The successful provider is expected to supply and manage Clinical Prescribers who will liaise with the ICB Protect NoW team, General Practice, and patients. A personalised care approach will be used to support pharmacological and social prescribing for our patient cohort.<br/>Prescribers must be able to initiate relevant medicines.<br/>Prescribers must initiate medicines in line with N&W criteria.<br/><br/>Patients will be identified using the ICB’s population health management software (Eclipse). The ICB Protect NoW Team will contact patients via digital (and print, if necessary) mediums to inform them of their relative risks and changes they can make in their lifestyles and diets and to encourage medication adherence where necessary.<br/><br/>Patients identified as eligible for medication will be consented by Protect NoW and subsequently made available to the provider, and the prescribers are to systematically contact this patient cohort and seek to initiate and / or optimise treatment in line with ICB and national guidance.<br/><br/>The provider will be expected to liaise with the relevant Practice where required on necessary ongoing management and monitoring.
