Description

The Pembrokeshire Integrated Assisted Discharge and Home Support Services aims to prevent unnecessary admission to hospital and facilitate early discharge from hospital and is a critical component of demand management from the perspective of the hospital and social care teams. The vision of the service is to utilise a range of third-sector intermediate care services, to provide a range of support, avoiding the need for conveyance to hospital and enabling people to stay safely in their own home.
The service is not a regulated care service and therefore is designed to meet non personal care needs based on a person-centered assessment of needs. The staff supporting the service and conducting the assessment not only consider the health & care needs, but also look at the wider determinants of wellbeing, signposting to third-sector services, addressing issues such as loneliness and social isolation, financial hardship, and so on. This service does not operate in isolation or seek to duplicate other third sector services in operation but aims to leverage support and refer to other services where appropriate.

Maternity and Neonatal Voices Partnership (MNVP) service

NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB) seeks to commission a Maternity and Neonatal Voices Partnership (MNVP) service for the BNSSG Local Maternity and Neonatal System (LMNS). The MNVP will act as an independent service user-led partnership, working alongside maternity and neonatal providers to gather, analyse and present the experiences of women, birthing people, babies and families, and support continuous quality improvement and co-production. The service will involve structured engagement, outreach, data collection and analysis, representation at LMNS governance meetings, and collaboration with maternity and neonatal clinical teams across the two acute Trusts within BNSSG. The service will deliver against national MNVP guidance and required outcomes set out by NHS England. The appointed Provider will employ a small core MNVP team (Trust Lead, Neonatal Lead and Engagement role) and provide the infrastructure, governance, supervision, digital systems and support required to deliver a robust and representative MNVP service. Key components of the service include: • Engagement activity across Bristol, North Somerset and South Gloucestershire. • Culturally safe, trauma-informed and inclusive approaches tailored to diverse communities. • Strong representation of Black, Asian and minoritised voices, deprived communities, and seldom-heard groups. • Collection, analysis and thematic reporting of qualitative and quantitative feedback. • Co-production of improvement activities with families and system partners. • Active attendance at LMNS governance, safety and quality forums. • Support for MNVP volunteers. • Social media and communications activity to promote engagement. • Applicants should demonstrate how they will support workforce wellbeing and contribute to wider system social value obligations, in line with NHS England requirements. • The Provider will be required to deliver a mobilisation plan including key milestones, risks and mitigations. The MNVP Strategic and Commissioning Lead will remain employed by BNSSG ICB and will work closely with the commissioned provider. Due to the changes in the NHS landscape, BNSSG ICB reserves the right to include NHS Gloucestershire Integrated Care Board to join the successful supplier contract during the duration of the contract if it is appropriate to so. BNSSG ICB reserves the right under Regulation 41(6) to directly award a subsequent contract under Schedule 5 section 8 (a). This procurement is being carried out by NHS South, Central and West Commissioning Support Unit.
