Description

The Pembrokeshire Integrated Assisted Discharge and Home Support Services aims to prevent unnecessary admission to hospital and facilitate early discharge from hospital and is a critical component of demand management from the perspective of the hospital and social care teams. The vision of the service is to utilise a range of third-sector intermediate care services, to provide a range of support, avoiding the need for conveyance to hospital and enabling people to stay safely in their own home.
The service is not a regulated care service and therefore is designed to meet non personal care needs based on a person-centered assessment of needs. The staff supporting the service and conducting the assessment not only consider the health & care needs, but also look at the wider determinants of wellbeing, signposting to third-sector services, addressing issues such as loneliness and social isolation, financial hardship, and so on. This service does not operate in isolation or seek to duplicate other third sector services in operation but aims to leverage support and refer to other services where appropriate.

Mental Health Supported Housing

LBH Joint Commissioning is seeking to commission a specialist mental health supported accommodation service for vulnerable adults in Hounslow. The service will operate across two buildings in Hounslow and a total of 34 units of accommodation. This service was originally part of a framework tender which went out in August 2025, it is being re-tendered as a standalone contract with a revised requirement. The eligibility criteria have been broadened to include experienced providers of mental health supported accommodation who are eligible for exempt accommodation rates of Housing Benefit i.e. Registered Providers of Social Housing, Registered Charities or Community Interest Companies. You will need a valid registration number to demonstrate at least one relevant status. The service will support adults with severe mental health problems who are referred by London Borough of Hounslow, offering 24-hour supported housing with flexible, recovery-focused support. The main role of the service is to provide supported move-on accommodation for individuals within the target group who are ready to take a first step down from a more intensively staffed setting. This will usually be from a community-based service, such as 24-hour supported living, but could also be hospital, provided an individual will be able to manage within the level of support provided by the service. In a smaller number of cases, appropriate individuals may be referred to the service as a step up from the wider supported accommodation pathway, mainstream housing or temporary accommodation. The service will promote recovery, wellbeing and independence and meet the following aims as detailed in the Core Specification: Maximise Independence Improve Mental and Emotional Wellbeing Enhance Quality of Life and Reduce Social Isolation Improve Physical Wellbeing Move on at the right time Prevent placement breakdown
