Description

North Wales Together: seamless services for people with learning disabilities invites submissions to deliver a 5-stage supported employment / specialist job coaching services to individuals aged 14 plus with diagnosed learning disabilities. The supported employment service(s) are a key part of the of the North Wales Regional Supported Employment Model which has been developed to respond the North Wales Supported Employment Strategy for people with learning disabilities. The model is being implemented within each of the six North Wales County Councils social work service for people with learning disabilities. The aim of the model is to establish access to paid employment as new care pathway within these services and thereby provide individuals with learning disabilities equal opportunity to be in paid employment based on informed choice.

The Service will support vulnerable young people who are making the transition from a care to a support setting and who may need varying levels of support, guidance, and input from specialist teams.
