Description

The purpose of this procurement is to provide a Social Wellbeing Service - The Service will operate within the boundaries of Wakefield District with a particular focus on Wakefield residents with a targeted approach towards communities of geography or interest who may be at greater risk of ill health through for example increased levels of deprivation and/or vulnerable groups or characteristics.

The Service will deliver the following direct measurable outcomes to improve health and wellbeing in the local population as a whole:

• Self-reported health and wellbeing is improved

• People are equipped with the confidence and tools to have a good life

• Knowledge and uptake of services to increase wellbeing, self-management and independence is increased

• People feel less isolated and have more opportunities for social interaction and participation

• People stay independent for longer due to increased access to information and better coordinated services

Additional information: An Open - Above Threshold, Light Touch Procurement process has been undertaken to appoint a service provider for the Social Wellbeing Service for an initial period of 4 years to commence on the 1st October 2023 with an option to extend for up to a further 24 months.
