Description

Southend on Sea City Council is carrying out this procurement exercise for the provision of supported living with outreach services Lot 1 and without outreach services Lot 2 and invites electronic tenders to join an Approved Provider List. The purpose of supported living is to provide support and care as needed to allow people to live as independently as possible within the community, based on the following principles: \r
1 all services are commissioned on the person centred basis and are required to be responsive to personal needs and preferences of each individual \r
2 given the right support, people with learning disabilities, mental health, autism, physical disabilities, or impairments can live well and to maintain their health with varying levels of independence\r
3 the good health and wellbeing of the people of whom these services are commissioned is everyone's business and not influenced by the type of organisation providing the service i.e. social care services. Further information is set out in the tender documents.

Day, Evening, Weekend and School Holiday provision for CYP accessing Services in the Disabled Children's Team

The Disabled Children's Team (DCT) works with children and young people who have diagnosed disabilities or health conditions which have a significant impact on their day to day lives from birth through to 18 years old when they transition to Adult Services. The Council would like to commission a new service to support individual CYP that access DCT. The aim of this provision is to ensure that disabled children and young people have access to social experiences and interactions at home and within the local community. Families are also provided with a break from their caring responsibilities. The purpose of this service is to provide an opportunity for children / young people with a disability or additional need to spend time away from their primary carers, exploring a range of new opportunities that are enjoyable to them. For example, disabled children and young people experience new relationships, environments and participate in positive activities. The ethos of this service is to match carers with individual CYP and their families with the view that over time, working relationships between the parties strengthen. The care for the child is consistent and outcomes within the CYP care plan can be achieved together. This service will offer specialist care when required; staff will be sufficiently trained with expertise/experience in working with disabled children and young people with SEND, some of whom may have multiple complex needs. The service has been designed so that the provider can train and support a growing workforce of care workers. Once fully operational, the service will be open to up to 35 families and will take place in the local community or in the family home.

Provision of a Foster Panel Chairperson

Caerphilly County Borough Council, Social Services Department (The Council) is tendering an opportunity for the provision of a foster panel chairperson who is a suitably qualified individual (the Service Provider) to act as the Independent Chair of the Council’s Foster Panel in line with the requirements set out in PART 5 of The Regulated Fostering Services (Wales) Regulations 2019. The membership requirements, terms of reference and functions of fostering panels are set out in: The Regulated Fostering Services (Services Providers and Responsible Individuals) (Wales) Regulations 2019; Regulation and Inspection of Social Care (Wales) Act 2016; The Fostering Panels (Establishment and Functions) (Wales) Regulations 2018; the National Minimum Standards for Fostering Services 2011 and; the Children Act 1989 Guidance and Regulations Volume IV - Fostering Services 2011. Aims of the Service The overall aims of this provision are: • To Chair the Council’s Fostering Panel. • To ensure that the Panel remains in line with regulations, policy and legislation. • To ensure the Panel members are aware of their roles and to facilitate the involvement of all Panel members. • To make clear, evidenced recommendations. • To ensure the Panel is efficient and professional, and to provide clarity where required.
