Description

Participants will develop key life skills through engagement in these, including personal responsibility / accountability; team working; leadership; communication and organisation skills.
There will be a nomination process for participants and the local community to suggest potential projects in conjunction with a network of partners The core tenet of this activity is 'co-design' and 'participant-led' to drive personal responsibility. Participants will take the lead in selecting, designing and delivering nominated projects/activities. Projects will be centred around other activities taking place in the Hub supported by partners. Workers will provide informal and ongoing information, advice and guidance as part of their ongoing engagement and motivational support .Bespoke tailored sessions will be designed for our participants, considering the often-unique set of circumstances that prevent them from accessing employment support.

Flintshire Dementia Community Engagement Service / Gwasanaeth Ymgysylltu Cymunedol Dementia Sir y Fflint

Flintshire County Council would like to appoint a trusted service provider to deliver a Dementia Community Engagement Service. The commissioned service will implement a dedicated post or small team to engage with the community of Flintshire, providing information and support relating to dementia. This service will lead the development of dementia friendly communities and information sharing networks to increase community participation and public awareness. This engagement service will complement statutory health and social care services, and commissioned services including the memory support pathway, and dementia unpaid carers support service. This service will be funded by Welsh Government Regional Integration Funding (RIF) and will replace an existing grant funded project that delivers similar service outcomes. There will be a requirement for Transfer of Undertakings Protection of Employment (TUPE) impact to be managed between the existing and commissioned providers. The initial contract will be for eighteen months with an option to extend for an additional two years subject to the continuation of Welsh Government funding. Flintshire Social Prescribing Service / Gwasanaeth Presgripsiynu Cymdeithasol Sir y Fflint

Flintshire County Council would like to appoint a trusted service provider to deliver a Social Prescribing Service. The commissioned provider will establish a social prescribing team working collaborative within the multi-agency Single Point of Access (SPOA). The commissioned social prescribing team will use specialist community knowledge to provide information, referrals and signposting to connect people to community groups, activities and assets that will meet their individual needs. The service will help to build the resilience of people and communities and will respond to the high demands for quality acute health and social care services. This service will be funded by Welsh Government Regional Integration Funding (RIF) and will replace an existing grant funded project that delivers similar service outcomes. Provision of Talking Lincs Advocacy Service

The Talking Lincs Self Advocacy Service will empower adults with learning disabilities, learning difficulties, autistic people and the neurodivergent community, those living with mental ill health and carers to speak up for themselves and their peers with the aim of improving the lives and services offered to people within North Lincolnshire.  To achieve this, we need to be able to hear the voice of people with lived experience and understand from them the issues they face in their daily lives.  This approach will support and influence the development of services across the council and partner organisations.
