Description

The Commissioners, Coventry City Council and NHS Coventry and Warwickshire Integrated Care Board,  are looking to contract with two providers for the delivery of long term home support for wards Holbrook and Radford; cluster area 1.  The contract will support adults in the provision of personal care and support which will be delivered in people’s own homes.  Support will be delivered by professional care staff to meet the requirements of individual support plans.  This will be based on eligible needs and aimed at supporting people to be independent and to achieve their maximum potential.  The service will also assist carers and parents to maintain their caring role.  The service will be for people that meet national eligibility criteria for social care and people eligible for non-complex health funded Continuing Health Care at home.

TD2320 - Family Group Conferencing

Derby City Council is undertaking this procurement to establish a contract for provision of Family Group Conferencing. The initial contract period is 1 year. The contract is estimated to commence 1st March 2026 and expire 28th February 2027. There will be an option to extend the contract for 12 months, subject to satisfactory performance and business needs. Maximum contract period is 2 years. There is a maximum budget of £90,000 per annum, a total of £90,000.00 over the initial contract term. The service will: • To facilitate and deliver high quality Family Group Conferences (FGC) in Derby, in accordance with the identified policies, practice standards and aims of the Family Group Conference Service. • To ensure that the service delivered is effective, of exceptional quality and value for money. • To ensure that preparation for the FGC reflects any existing formal or statutory plans in place (e.g., Child Protection Plan, Child in Need Plan). • To fully include the voice of children, young people and their families in decisions relating to the services being provided. • Ensure that the Family Plan is child focused, does not compromise the safety of children and meets the expectations of the referring agency. • To improve outcomes for families in Derby City.
