NHS South West London Integrated Care Board (ICB) (Croydon Place) is seeking to commission Long-Term Conditions (LTC) Community Outreach, Expert Patient Programme (EPP), LTC Self-Management and Pentathlon Healthy Lifestyle Programme Services. Since 2020, the LTC Community Outreach and Expert Patients Programme has been focussing on prevention, early identification and improved management of LTCs in Croydon with the aim to raise awareness with people who do not regularly access GPs or local NHS services. With the population in Croydon with two or more long term conditions at 23% and set to increase, a collaborative approach across all long-term conditions is key to manage, support and prevent increases in the number of Long-Term Conditions and the costs associated with that to health and the individual. The aims of the LTC community outreach and EPP programme are to deliver cost-effective, culturally competent, and preventative community health interventions that address the health inequalities in Black, Asian and Minority Ethnic (BAME) communities which constitute circa 50% of SWL ICB's core20 communities, particularly in deprived and underserved wards such as Thornton Heath and New Addington. In particular the LTC community out-reach health and wellbeing checks, Pentathlon healthy lifestyle courses and Expert Patient Programme (EPP) LTC self-management course are anticipated to: • Support raising grass roots awareness of LTC risk and risk factors and support people to adopt healthier lifestyles to reduce the risks of LTC, support early identification and diagnosis and self-management in key Croydon communities. • Empower patients with LTCs to feel more confident in managing their own condition and keep them active and engaged in their lives. • Tackle health inequalities and increase uptake from hard-to-reach groups. The Contract is expected to commence in April 2026 for an initial period of 2 years. The contract may be extended for an additional period of 1 year at the sole discretion of the Authority. The total contract value for the 2+1 period is £297,000 inclusive of VAT. (£99,000 inclusive of VAT per annum) Interested Organisations must express an interest via Atamis e-tendering portal. NHS Midlands and Lancashire Commissioning Support Unit (CSU) is acting as an agent on behalf of another NHS Body - NHS Lancashire and South Cumbria Integrated Care Board (ICB) who wishes to procure Community Services for West Lancashire. NHS Lancashire and South Cumbria ICB has set out an ambition to have a world-class, community-centric health and care system, with its four place-based partnerships, or 'places', at the heart of this Transformation. One of the ICB's top priorities is to transform community health services and integrated community teams, aiming to elevate health outcomes, tackle health inequalities, and keep more people thriving within their communities. By transforming community services, the Lancashire and South Cumbria system will be empowered to support people in staying well, proactively identifying those at risk of deterioration, and offering them personalised care and support. Community health services cover a wide range of services and provide care for people from birth to the end of their life. Community health teams play a vital role in supporting people with complex health and care needs to live independently in their own home for as long as possible. Lancashire and South Cumbria has four places: Lancashire, South Cumbria, Blackpool, and Blackburn with Darwen. West Lancashire is a district area of Lancashire and is predominantly a rural district. It has a population of approximately 118,000 people and is made up of the 1960s new town of Skelmersdale, the historic market town of Ormskirk, and a number of villages situated primarily in the rural Northern Parishes. West Lancashire Community services are mainly adult physical health services but include paediatric podiatry services for those children who need local access to community services. The current provision will conclude at the end of March 2026, and this notification is provided to enable interested parties to plan for a potential Provider Selection Regime (PSR) Competitive Process. The value of the contract will be £10,997,183 per annum. The total contract value over the 3 years plus the potential 2-year extension will be £54,985,915.00 There is a range of specifications that will form the overall contract and these include a Community Diabetes Service; a Community Stroke and Neuro Rehabilitation Service; and an Intermediate Care Therapy Service. The ICB is looking to award one contract for all of these specifications.
