Description

Contract for the provision of Accommodation and Support to citizens who are homeless at the point of discharge.  This will include but not limited to:•	Provide good quality accommodation that meets the needs of the citizens including those who are bariatric, and wheelchair users.•	Provide flexible person-centred support ensuring that support is delivered in response to the client’s needs and allows them to maintain accommodation and improve their health and wellbeing outcomes.•	Provide the lead and support which will assist clients to navigate and gain access to appropriate services, including but not limited to health care, substance misuse, education and/or support to gain employment.The Contract will last for 5 years (unless terminated under the terms and conditions of contract) with the provision to extend for up to an additional 2 years subject to requirements, funding and satisfactory performance.

AMCV 333 - Barleyfield House Respite Provision for Adults with Learning Disabilities and Autism

Shropshire Council is dedicated to providing high-quality residential respite care for adults with learning disabilities, supporting families in their caregiving roles. The current contract for residential respite care services is set to expire in September 2025. The home, located in Shropshire, is registered with the Care Quality Commission to accommodate up to four residents. It primarily offers planned respite care but is also equipped to handle emergency respite requests at short notice. All places within the home are contracted by Shropshire Council. The property is leased by Shropshire Council, and the successful tenderer will be required to enter into a license agreement or equivalent to cover the occupation of the home. The form of the license agreement is included with the tender documents. Shropshire Council invites tenders from providers for the care and support element of this service, including some hotel costs as outlined in the Finance Model schedule. Shropshire Council will act as the Commissioning Authority for the tender process. If the successful tenderer is not already registered with the Care Quality Commission, registration will be required following the award of the contract. The contract will be let for one year, with an option to extend for up to a further two years. The terms, conditions, and standards are contained in the enclosed sample form of contract, which will be reviewed periodically to ensure it includes the Council’s up-to-date contract clauses. Shropshire Council is tendering this service to meet the ongoing requirement of demonstrating Best Value. Providers tendering for these services are expected to identify improved efficiencies while maintaining a high-quality service. The Council seeks to appoint an innovative provider who: •Can meet the varied needs of up to 40 different residents. •Can develop the current service in line with the Council’s priorities. •Has a track record of providing high-quality residential respite services of a similar nature. •Has proven abilities to think imaginatively about service development and improvement. •Can demonstrate an awareness of local and national agendas for both health and social care. Performance of the home will be monitored and contribute to the Council’s overall performance framework. Young People’s Housing Support Services for 16–25 year-olds - 2 LOTS

CONTRACT FOR THE PROVISION OF YOUNG PEOPLE’S HOUSING SUPPORT SERVICE Wandsworth Borough Council is inviting tenders from suitably experienced and competent providers for the provision of Young People’s Housing Support Services to eligible vulnerable young people in Wandsworth aged between 16 to 25 (16-25) years old. Housing Support Services (HSS) assist the Council in providing access to supported accommodation, which is crucial for offering safe, stable, and suitable housing for our vulnerable young homeless people, including care leavers, single homeless people, young parents and young families. HSS help young people to develop practical life skills that will facilitate their transition into independent living, adulthood, and social inclusion in a structured and supportive manner. It is important to have a provider with extensive experience in housing-related support or specialist young people's services, particularly those familiar with services for care leavers, Ofsted regulations and trauma-informed approaches. The Council aims to commission two housing related support services for young people in 14 shared properties across the borough. The procurement will be split into two (2) Lots as set out below: Lot 1: Young People’s Housing Support Service (47 Units); Lot 2: Young People’s Housing Support Service (24 Units); Providers can apply for both Lots, but only one Lot will be awarded to each provider. A provider can be awarded either Lot 1 or Lot 2, but not both accommodation-based housing support contracts. Contracts for both Lots will be for a period of five (5) years commencing on 1st October 2025 and subject to an extension of a period or periods of up to an aggregate of two (2) years at the absolute discretion of the Council.
