Description

The first strand Service is a short term rehabilitation, reablement and assessment service for Service Users at risk of requiring long term residential/nursing care or to prevent hospitalisation. The second strand is to promptly facilitate the discharge from hospital of in-patients or those presenting for emergency care who have been assessed as clinically fit for discharge but who may, following a further home based assessment, be found to have a need for reablement or rehabilitation and a requirement for short term or longer term support in order to remain at home.
In both cases the Provider will work closely with the Service User, family carers, Occupational Therapists, Physiotherapists, other health professionals and known community networks to facilitate a support plan that allows Service Users to remain within their homes.

Workforce planning in a neighbourhood or place

The Governments 10 year Health Plan aims to deliver three shifts – hospital to community, analogue to digital and sickness to prevention - that will ensure that health and social care are fit for the future. A well resourced and supported workforce will be central to government delivering these shifts. Planning for a workforce to deliver these shifts in a neighbourhood or place requires different skills to traditional workforce planning and not all councils yet have the skills or confidence to do this. Requests for support from PCH for strategic workforce planning have increased since 2023, including requests for support at regional as well as placed based level. DHSC require PCH to prioritise support to those councils being assessed as ‘inadequate’ or ‘requires improvement’ in the CQC assessment process. Many of these councils identify workforce planning as an area for improvement under the ‘providing support’ theme. A contractor is being commissioned to work with PCH to provide targeted support to councils, neighbourhoods or places to develop impactful strategic workforce plans that will drive improvement locally and support the delivery of the governments three shifts. The objective of the support is to help up to 15 places, which may be regions, individual councils, neighbourhoods or places defined by local health and care leaders in ICB’s, to develop a strategic workforce plan that will bring impactful change. This will include addressing short term workforce challenges as well as considering longer term transformation to move towards one workforce in a place. Local Healthwatch Services for East Berkshire

Slough Borough Council, Royal Borough of Windsor and Maidenhead and Bracknell Forest Council (East Berkshire) are tendering for a provision of a Local Healthwatch Service for East Berkshire. The service will cover all three local authority areas under a partnership arrangement with Slough Borough Council being the lead contracting authority. Local authorities have a statutory duty to commission a local Healthwatch service (LHW), under the Health and Social Care Act 2012, the statutory activities are: to undertake, such as gathering local views and making these known to providers and commissioners, monitoring and scrutinising the quality of provision of local services, and a seat on the local health and wellbeing board. The requirement for local authorities who commission Adult Social Care services to commission a local Healthwatch that is independent of the Local Authority. This service will be carrying out the statutory duties that a Healthwatch services provides. Local Healthwatch Service (LHW) is the independent consumer champion for health and social care on the local level. It: • Represents the collective voice of residents, patients, carers and the public (including children and young people) to commissioners, service providers and local politicians• Acts as a focal point for the local community to have a voice in the commissioning and provision of health and social care• Provides opportunities for local people of all ages to influence decisions being made about their services, across the NHS and social care. Slough, the Royal Borough of Windsor and Maidenhead (RBWM) and Bracknell Forest are unitary authorities in Berkshire, South East England, at the heart of the Thames Valley. These three unitary authorities are located within the Frimley Integrated Care System (ICS). The contract duration will be for three years with an option to extend for one + one year(s) (3+1+1). Wiltshire Council recognises the impact that poor mental health can have on an individuals’ ability to maintain employment and acknowledges the need for new and innovative approaches to improve mental health in the workplace. Sustaining Work will focus on employees in the workplace who are struggling with their mental health and wellbeing by providing support and interventions that will reduce the risk of them not being able to sustain working lives. To focus on mental ill-health support for employers and employees with mental ill-health interventions and learning and development opportunities to help people achieve regular incomes and sustain work. It is recognised that to do this innovative approaches are required to improve mental health in the workplace. The service will primarily be aimed at businesses in Wiltshire, with a focus on small and micro businesses that may not have the internal infrastructure, such as a HR department or an occupational health team, to support staff in this way.
