Description

East Sussex County Council and Brighton and Hove City Council has awarded to  single provider to manage a SEN and disability advice service ("the Service") in accordance with the SEND Code of Practice, and the Minimum Standards for SEND Information, Advice and Support Services. This will secure the provision of impartial, confidential and relevant information, advice and support to: the parents and carers of children and young people between 0 and 25 years who have, or may have, SEN or a disability, and children and young people who have, or may have, SEN or a disability.

Re-Opening of Mental Health Independence and Support Framework

The purpose of this framework is to offer support to those people living in Sheffield who have a severe mental health problem that is having a significant effect on their health and wellbeing. Mental health needs are increasing, economic and environmental factors are taking their toll. The aim of this framework is to address some of these issues before they can develop, offering support at the right time and enabling a return to independence as quickly as is possible. For others it may be ongoing support at a level that allows as much autonomy as the individual can manage. Services will be available to individuals who reside in Sheffield who have a diagnosis of severe mental illness and have eligible needs under the Care Act 2014. Once an individual's needs have been assessed as being eligible for Social Care funding, they will be allocated a package of support. This support must meet the needs set out in the Care Act (2014) Assessment or Section 117 assessment. It is the responsibility of the assessor to decide what level of support will be required for each individual. This decision will benefit all parties who are then able to understand and tailor support to the individuals' current abilities and needs. The level of support will be determined at assessment stage and will fit into one of the two categories: LOT 1 - Standard Support 1.1 Outreach 1.2 Supported Accommodation LOT 2 - Enhanced Support 2.1 Outreach 2.2 Supported Accommodation
