Description

Forty (40) Contracts have been awarded for the provision of Personal Medical Services (PMS) Commissioning Intentions services in West London boroughs. The aim is to provide services to patients registered with West London GP practices. The term of the contract is from 1st July 2022 for 3 years with the option to extend by a further 2 year period. The estimated total annual budget of all the contracts is £5,931,618.<br/> <br/>NHS NW London is required to reinvest monies released from the PMS review under the national terms and requirements of the PMS review and these monies are furthermore required to be invested into GP practices within the borough from which the monies have been released.

Harrogate Court Extra Care Scheme

The City of Doncaster Council is seeking a suitably qualified and experienced provider to deliver care and support to tenants of Harrogate Court along with housing related and general wellbeing support and a community hub, providing a high quality service at all times. Care and support packages will be delivered in line with assessed need. It will be a 24/7 service including first response. The contract will be for a period of five years with the option to extend for a further five 12 month periods. Harrogate Court is a 50-bed apartment extra care scheme, for People aged 55 and over. The scheme has 21 one-bedroom apartments and 29 two-bedroom apartments. All apartments are for rent. (There is also a communal lounge, laundry room, hairdresser, restaurant , shop and guest room. It is a requirement of this contract that you hold a CQC rating of good or above from the start and for the duration of this contract. Harrogate Court is not currently registered with the Care Quality Commission (CQC), on contract award the successful provider must commence the CQC registration process. Until the service and manager are both registered with CQC, the successful provider must ensure that the service is overseen by a registered manager or CQC nominated individual, to provide consistent support to the manager. The nominated individual or registered manager designated must currently be responsible for a care service which is currently rated as ‘Good ‘or above with CQC.
