Description

The Work and Health service delivers a job retention service linking to GP practices. The main objective is to provide support and advice for patients whose health is affected by their work and to help and support them to keep their job. This is provided through advice on employment and health matters such as preventative advice, return to work advice and benefits advice.
The budget envelope for this project is £98,000.00 per annum with a total value of £294,000.00 for the initial 3 years and a total value of £490,000.00 if the two year extension is executed.

Extra Care Housing Schemes in Slough - Care and Support Services

Slough Borough Council is seeking suitably experienced and qualified providers for the provision of Care and Support services for Extra Care Housing across two schemes in Slough, at The Pines and Northampton Place (total 126 units). The Care and Support services offered in Extra Care are mainly aimed at older people but may also meet the needs of younger adults with physical disabilities, learning disabilities or mental health challenges. The service will operate a Core and Flex model: a guaranteed on-site core (including a waking night presence and a light daytime cover window for unplanned response/coordination) with flexible, planned care commissioned on actuals to meet individual outcomes. The contract will commence on 1 September 2026 for an initial three year period with the option to extend for two x twelve month periods to 31 August 2031. The Connect to Work programme aims to support 1565 economically inactive residents in West Northamptonshire over the next five years by delivering personalised support, including employability training, confidence building, and workplace adjustment advice, increasing awareness and access to local job opportunities, identifying and addressing barriers to employment for people with disabilities and long-term health conditions, reducing economic inactivity and reliance on other services through sustained employment, promoting diversity and inclusion among local employers and working collaboratively with other South Midlands local authorities. This funding is allocated to the South Midlands Partnership region. WNC is the accountable body for Bedford Borough, Central Bedfordshire, Milton Keynes, North Northamptonshire, and West Northamptonshire. This tender relates specifically to West Northamptonshire.
